HARM REDUCTION THERAPY FOR COMPULSIVE SEXUAL BEHAVIORS IN CALIFORNIA

Your relationship with sex and substances is complicated. You deserve a therapist who can hold both.

CAHRT therapists offer non-judgmental support for compulsive sexual behavior, chemsex, and sex and drug-linked patterns. No shame, no labels, no abstinence required. California and telehealth.

Specializing in Sexual Compulsive Behaviors + Substance Use

Many people who come to CAHRT have previously seen therapists who were great with mental health but didn't understand substance use. Or counselors focused on addiction who couldn't talk about sex without pathologizing it. Or clinicians who treated compulsive sexual behavior as a moral problem rather than a human one.

CAHRT providers who specialize in compulsive sexual behaviors bring a harm reduction framework to work that is often stigmatized, rarely understood, and deeply personal. No labels. No shame. No abstinence requirement.

Shame doesn't create change. It just makes it harder to ask for help.

Compulsive sexual behavior, sex and drug-linked patterns, and chemsex are areas where stigma runs deep and good clinical care is genuinely hard to find.

People navigating this often feel like they are the only one. They worry that talking to a therapist will mean being labeled, pathologized, or told that their desires or behaviors mean something is fundamentally wrong with them. They may have had those experiences before.

Harm reduction therapy approaches this work differently. The goal is not to fix you or define you. It is to help you understand your own patterns, make sense of what they are doing for you, and figure out what you actually want, on your own terms.

Harm reduction for compulsive sexual behavior: what it looks like.

CAHRT has several providers who specialize specifically in this area. They bring deep experience working with the full range of compulsive sexual behavior, sex and drug-linked use including chemsex, and the complex intersection of identity, desire, substance use, and mental health that often shapes these patterns.

This work can include:

  • Exploring the role sexual behavior plays in your life without judgment or a moral framework

  • Understanding sex and drug-linked patterns, including chemsex, and what drives them

  • Working with shame, secrecy, and the isolation that often accompanies compulsive behavior

  • Setting goals around behavior that are yours, whether that's changing how you engage, using more safely, or simply understanding yourself better

  • Holding your sexuality and your substance use together in the same room without either being treated as the real problem

CAHRT providers in this area are LGBTQIA+ affirming and are familiar with the specific contexts, communities, and cultures in which sex and substance use are often intertwined.

Frequently Asked Questions About Harm Reduction Therapy for Sexual Compulsive Behaviors

Is compulsive sexual behavior a real clinical diagnosis?

Compulsive sexual behavior disorder was added to the ICD-11 in 2019. However, there is ongoing clinical debate about how it should be understood and treated. CAHRT providers approach this with nuance, neither dismissing the reality of someone's distress nor over-pathologizing behaviors that may be consistent with healthy sexuality in context.

Do I have to stop the behavior to work with a harm reduction therapist?

No. Harm reduction therapy does not require behavioral abstinence as a starting condition. The work begins with understanding, not stopping, and goals are set collaboratively based on what you actually want.

What is chemsex and can CAHRT providers help with it?

Chemsex refers to the use of specific substances, often methamphetamine, GHB, or mephedrone, in the context of sex, most commonly within gay, bisexual, and queer communities. Several CAHRT providers have specific experience working with chemsex and bring a harm reduction, LGBTQIA+-affirming approach to that work.

What if I'm not sure whether my behavior is a problem?

That uncertainty is a completely valid place to start. Harm reduction therapy can help you explore your own patterns and figure out what feels harmful and what doesn't, without someone else deciding the answer for you. You don't need to have a diagnosis or a crisis to come to therapy.

WHAT HARM REDUCTION THERAPY ACTUALLY IS

This might be for you if...

  • You feel shame about your sexual behavior and you've never found a therapist you could actually talk to about it.

  • Your drug use and sexual behavior are connected and you've never found a provider who could hold both.

  • You've been in chemsex patterns and you want support that understands that world without judging it.

  • You're not sure whether what you're experiencing is a problem or just who you are, and you need space to figure that out.

  • You've been in LGBTQ spaces that push sobriety, or sober spaces that aren't queer-affirming, and you need somewhere that holds both.
